How to activate a protocol via the CTN?

1.) Review the protocol as posted in on the CTN protocol or on the cooperative group webpage.

2.) If your site wishes to pursue the study, submit a study request form to the Network Office.

3.) A confirmation letter will be submitted that outlines all of the items needed for protocol activation. In this packet, the sub-contract will be provided.

4.) Obtain IRB approval.

5.) Executed the study specific contract.

6.) Submit all activation document to the Network Office.

7.) HCC will issue an activation letter signifying that protocol enrollment can begin.
